Heritage Program for Seniors

The Heritage Program for Senior Adults is our outpatient program which provides personalized treatment for older adults dealing with emotional, behavioral, or mental health challenges. This intensive outpatient program operates Monday through Friday, allowing patients to return to their homes in the evenings.

The outpatient program assists seniors facing various issues, including:

  • Transitions in life
  • Grief and loss
  • Medication education
  • Anxiety and worry
  • Relationship concerns
  • Depression
  • Age-related memory changes
  • Anger management
  • Stress reduction techniques
  • Changes in eating and sleep patterns

 

Most therapy sessions are conducted in group settings, although individual therapy is available when clinically necessary. These groups are skillfully led by Masters Level Clinicians, under the supervision of board-certified Psychiatrists. Furthermore, we prioritize collaborative care by coordinating treatment with patients’ families, caregivers, and physicians to ensure the most effective solutions for each individual.

Treatment Goals– Our overarching goals are to enhance daily living skills, alleviate symptoms hindering individual functioning, preserve family integrity, and promote maximum independence.

Assessments and Referrals– Our clinical staff at the Heritage Program is pleased to offer confidential assessments, free of charge, to determine the need for behavioral health services. Referrals to the Heritage Program can be initiated by physicians, nurses, social workers, or any concerned individuals.
